Some things that you need to take care of before coming to Tufts:

  1. Physical Exam
  2. Immunizations
  3. Health Insurance
Necessary forms for incoming freshmen are due by July 15, 2009.

PHYSICAL EXAMINATION

Required for all students. Download the health examination form.

-TB Risk Assessment required for high risk students: (see 'TB risk assessment questionnaire' on your health examination form): Important: Clinician must sign the TB risk assessment located on your health examination form!

-High Risk Students:
  • PPD (mantoux test) required for students who may be at high risk for Tuberculosis (TB).
  • Please review immunization section of the medical form for specific details on risk factors.
  • One of the major risk factors for TB is having lived or worked in areas of the world where TB is common: Asia, Africa, South America, Eastern Europe, Russia, etc. All students coming to Tufts from one of these countries must show documentation of TB testing done within 12 months prior to entering Tufts University.
  • Previous BCG vaccination is not a contraindication for skin testing.
  • If the PPD is positive (10mm or more of induration), a chest x-ray is necessary.
  • If the PPD is contraindicated because of a previous positive test, a radiologist’s report of a chest x-ray within the past 12 months is necessary.
  • If the student has been treated for TB, a chest x-ray is not needed, but information about treatment must be included.
-Low Risk Students:
  • No testing required. Clinician still must sign the TB risk assessment located on your health examination form!

IMMUNIZATIONS

-Measles, Mumps, Rubella (MMR): Two doses Required

  • Two injections of Measles and Mumps vaccine are required, given at least one month apart, with the first given at 12 months of age of after.
  • At least one dose of rubella vaccine is required.
  • Immunity to these diseases must be documented by immunization with the live vaccine givenin 1968 or after, or by serologic (antibody) titers. History of having had the disease will notbe acceptable for documentation of immunity.
  • If there is a medical contraindication to any of the above immunizations, please attach anexplanation and serologic antibody test result for the indicated disease(s).
-Tetanus/Diphtheria (Td or Tdap): Required
  • A booster injection of Td or Tdap is required within the past 10 years.
-Hepatitis B Vaccine: Required
  • Three doses of Hepatitis B vaccine, or positive serologic titer is required.
-Meningococcal Vaccine: Required
  • One dose of either the polysaccharide or conjugate vaccine within 5 years.
  • OR signed waiver that the student understands the risks, and elects to decline vaccination.

Your immunization information is a critical component of your medical records. Massachusetts State Law requires all college students to show proof of satisfactory immunizations. Students who have not fulfilled the state mandated health immunization requirements will not be permitted to register for the next semester classes until proof of documentation has been submitted.

HEALTH INSURANCE

Massachusetts mandates that all students have Insurance. Please go to www.aetnastudenthealth.com to look at the policy offered to Tufts students. Go to "Find Your School" at the very top of the page and look up Tufts. If you have comparable coverage you may waive the Insurance Charge that appears on your tuition bill. The waiver site will be accessible by July 8, 2009. At the top of the page it will say, click here to waive your Insurance Charge. You will need to complete the waiver and obtain a confirmation. Within ten days the fee will be removed from your tuition bill. Please note: You should complete this process by August 3, 2009. This waiver process needs to be done every year, until you graduate.
